Admission criteria

New Era accepts adult patients who are medically stable but require continued support before a safe return home. Referrals are reviewed individually — when in doubt, call and ask.

Medically stable, discharged or discharging from an acute care hospital
Requires short-term recovery support, wound care, or medication management
Lacks a safe or appropriate place to recover at home
Able to participate in daily activities with support (non-bedbound)
Willing to engage with case management and care planning
